 When submitting a player's name, please be sure to enter the full name as it appears on his or her passport. |
 Each team roster must include at least two females. Please specify M or F next to the player name. |
 Help us make the tournament's meals enjoyable for all players by consulting your teammates for food preferences. Use the check boxes to indicate vegetarians on your team. |
Under “Special Considerations” enter any other information that
will help the tournament planning committee better serve your
team’s needs. Please be sure to include unique travel plans
(i.e. players arriving earlier than Friday or departing later
than Sunday), special diets, and entry visa issues. |
